This is a canker treatment that a good friend of mine Dr. Andrew Watts, a veterinarian that has a special interest in Podiatry sent to me from Bahrain.

Here is my treatment review and recommendations, there are three key points. Clean it well daily, preserve ALL soft tissue(DO NOT DEBRIDE), and keep it dry.

In one month of treatment for the first time in 20yrs I was able to successfully treat in a reasonably short period a serious canker as follows. The critical treatment variation from traditional therapy is leaving the inflamed villi undamaged. In my opinion this is why the case responded well, I have always removed this tissue and I feel now that protecting this structure makes sense.

1. The hoof was trimmed carefully so no damage to soft tissue occurred.

2. The hoof was blocked then the lesion thoroughly cleaned with 5% Benzyl Peroxide cream, making sure to massage the cream deep into all areas. Once finished gauze was used to dry the lesion.

3. A mixture of crushed Metronidazol tabs was prepared. Pack the powder into the wound(there will be unavoidably some haemorrhage assoc.. with cleaning) and firmly bandage.

Change dressing daily. Surprisingly, the wound took approx one week of daily cleaning before there was no evidence of contamination. At this stage the villi had regressed and the lesion was reasonably dry and free of discharge and odour. I then added crushed Iodine crystals to the Metronidazol preparation, slowly increasing the concentration until by the 4th week the powder was almost dark chocolate in colour. The hoof was then trimmed and reshod as seen. The preparation was then changed to Sulprim powder mixed with a smaller amount of iodine to avoid excessive drying and cracking of the new sole.

Dr. Andrew does not shoe but he does QuixShoe. He supervises his Farriers well and is well versed in all podiatry issues. I worked with him for 2 years here in Dubai. He was so excited at the result on this horse, as he has like so many including myself battled with this dreaded disease.

I have one question, and the answer may be obvious but I'll ask anyway. The shoe that has the area cut out, was that applied before the surgery and cut away to perform the operation or was the horse shod immediately following the operation? It seems like it would make sense to shoe before the operation but I could be wrong.

David. not sure about the shoeing as this was not done by me but by a vet and farrier in Bahrain. This would not be classified as an operation as such, as the horse was nerved and then the lesion cleaned. Nothing was cut and the lesson learned by the Veterinarian was to be non-invasive when dealing with canker. Invasion means blood and lots of it and a long slow healing process, with mixed results.
What would be great is that other Vet's and Farriers around the world try this protocol and see if they get the same results.

Very nice and so glad it came out so well.

I worked with a local farrier on a cancer in the frog area of a percheron and he tried three different methods before cauterizing.

I love success stories....

Hello Derek,

I deal with any place from 6 to 12 canker infections a year.
We first try non-invasive methods on all cases.

The method we use is exactly the method you had posted. We clean with Benzyl Peroxide cream, we apply the crushed Metronidazol tabs. Owner changes bandage daily. This medical treatment can be found in an abstract written by Dr. Stephen O'Grady's for the 2004 AAEP.

I Have not used Iodine Crystals in many years and worthy of trying again.

For me, this method seems to work with some cases (about 30% or so). When we return in five weeks if infection is not cleared up then we schedule for debridement and use all of the above all over again.

That appears to work for the remaining cases except for a few cases in which there is no resolution and the infection is so deep and never clears up that the horse usually is euthanized. Several of these case photographs are posted on my Facebook page.

I do believe that O'Grady had a case or two that did not resolve and went on for laser photoablation. I have never had that opportunity and would like to know how the outcome was.

The pictures are great and thanks for allowing all of us to share in the happy ending.

Metronidazol has always been my treatment of choice after working with Steve O'Grady for many years.

Andrew found that by adding the iodine crystals it did dry and heal faster.
It was thanks to Steve's paper that Metronidazol is in use by hoofcare professionals the world over.

Was planning on the day off until I received a telephone call from a veterinarian to attend to another horse with canker.

In a telephone call afterwords to a friend, he asked me about some common denominators with canker infected horses. My observations are this: I've seen it in many breeds (drafts, quarter horses, warmbloods, ponies, Morgans, TB's, Arabs), wet and dry environments, well cared for and less cared for hoof care situations, clean and dirty environments....

The one thing I can say is common is nearly every canker infected foot that I have been part of is that the heels are contracted (a heel to heel width that is well inside the widest part of the toe), even in my draft horses. Only a few case have not.

What has been anyone else's observations?
