Interested in your thoughts on the RF. What has happened? Shoeing protocol? Prognosis?
-Aged upper level 18 hand dressage horse,
-outward deviation below the knee,
-intermittent medial RF quarter cracks for 10 plus years
-Cushing's diagnosed after laminar tearing noticed (past 12 months)
-major diet change + drugs
-RF some laminar tearing from ahead of lateral toe nail to 2nd medial nail hole
-LF normal laminar separation from 2nd toe nail to 2nd toe nail
-recent (4 months) RF quarter crack eruption.
-heart bar shoe with IM and floated medial quarter
-horse sore, owner suspected excessive frog pressure
-IM removed 2 weeks ago horse sound. NO depression below the coronary band
-horse is sound now
-horse wearing casts with IM until we decide on shoeing
Below is an older conformation photo. Hoof photos from yesterday.

Xrays with casts in place.

I agree with Rick, this is laminitis. Shoe accordingly, sound or not......

Is it possible the medial sinking is a result of floating the medial quarter for the quarter crack? Or do you think this has been present right along?

I agree with both assessments. Given the static pressure on the medial wall and the fact that the horse is sinking medially what type of shoeing would you use. It seems that the any of the normal laminitic shoeing protocols fail to address this particular horses needs. Horse is still sound and in work but we need to address these new issues.

Now there is a scary thought. I lower the lateral side on my horse to stop him from interfering. It's not more than a couple of extra swipes.

Should I be concerned with this as a possibility?

Good question, but only the heel quarter is floated and usually settles to the shoe within 24 hours. 2 weeks prior to change in coronary band the hoof was on the shoe.

Is the displacement "remodling" due to the limb deviation?

Maybe before Mr. Mickle approached the horse there may have been a case of 'RIGHT HANDED FARRIERS DISEASE". Notice the lack of pectrolal developement.
Use the "Whole Horse"..(David Gill). It ain't all in the foot people....

The limb deviation, quarter cracks, and Cushing's onset laminitis and medial sinking are givens at this point.
My thoughts are that the Cushing's is the main culprit and recently the horse had been experiencing sub-clinical laminitis of the medial wall consequently, moving static balance on the hoof laterally exacerbating the crack until within the past two weeks there was major medial laminar failure resulting in the sinking and hoof wall remodeling. This is just a theory I am interested in other thoughts and again a protocol to arrest the problem. Thanks to Pat Reilly who suggested doming IM under a cast. I wonder if this doming should be symmetrical given the structure of the leg.
Thanks for the input so far.

Jay, I am going to step out of my comfort zone and ask a question.

Jaye Perry noticed lack of pectorial development.
and right handed farrier disease.

You stated the hoof was floated and the other problems.

Here is my question.
Since this is medial, is it possible the horse is putting too much pressure in that area (possible due to body imbalance ?)
If you were to put a pad on the opposite foot and raise that leg up would it relieve ths stress on the injured and re-distrube the weight to the lateral side of the injured hoof, thus allowing the foot to heal and not be fighting the stress ?
In other words "tip the horse towards the lateral side of the inflicted leg...taking any body weight off of the the medial side.
In this way you could shoe with a bar shoe or shoe of your choice and even once again float the afflicted area...?(remembering to trim for the adjusted body positioning)

In stead of trying to correct the foot, alleviate the stress by repositioning the body of the horse.....

I think of problems as a battle... If there is more than one battle that the horse is coping with, eliminate a battle and the horse's body can then focus on the one that needs the most healing.

my 2 cents worth ;)

Im not Jay. But since there is no bone attachment in the shoulder I don't think it would tip the horse. Just raise the shoulder I slightly higher?
