Hey guys, been dealing with this case for a couple months. The horse first presented lameness in December (I was not the attending farrier until July) When I first showed up the horse was laying down everyday and would not spend a lot of time up. His right front was far more advanced than his LF. After the first appointment which we used a s1 glueon, hospital plate and IM the horse was much better and was walking. 3rd visit the horse was better again. after the 4th visit we tried a sigafoos roller shoe with the soft black pad and it failed causing sole pressure. Horse was very uncomfortable days after visit. Re-shod with open toe eggbars to lessen chance of sole pressure. He's doing better now.

This is what he currently wears.


Right now the damaged hoofwall has grown down to around the quarters so I have to rebuild it a bit every time with superfast to accommodate a shoe, otherwise the shoe would sit right on the sole.

The Vet wants to consider an Tenotomy. Im not sure how I feel on the subject. I dont think its needed considering I think we've got good laminar attachment, and most of the pain I believe to be sub-solar abscessing and not a stressed out laminar attachment or rotation. I was hesitant to go probing for abscesses last visit considering the sole and frog looked prolapsed and I was not sure about our sole depth. We shot a film that day and In fact I think we are pretty good on sole depth right now.

What I think we can improve upon is the trim and shoe placement and creating an angle that will better align the bone column.

As you know when things are a mess and you don't have updated films in front of you, some of us get a bit gun shy (for good reason) when attacking the foot. Unfortunately the vet who we are working with (who is a good vet and great to work with) does not have digital imaging.

I feel that if we can get the heels where they need to be and provide and shoeing package with good amount of break-over and possibly and roller motion shoe that will allow him to find a comfortable angle, we will be onto something. I had a similar package on him with a hospital plate that provided a lot of this, but the horse owners took it off becuase it was hard for them to deal with cleanign the bottom of the foot as the horse wont hold it up. I was pretty annoyed by this and have been avoiding using a hospital plate since.

This was that package.


I would probably be using a sigafoos shoe as an open toe eggbar and rails. But have been considering something like the JB open toe heartbar shoe with series 3 cuff and rails believing that the heartbar would give me support when the rails elevate the foot off the ground.

Why tenotomy? The pastern axis has a good line. It looks a capsular rotation.

I like your treatment for this case.

A horse trainer once told me that training horses was getting them to yield to pressure. Likewise, treating founder cases is about loading the heels.

By lowering the heels more, you are loading the heels more. Raising heels where P3 is not properly derotated/realigned (or whatever else you could call it) will increase pressure in the anterior part of the foot, éven when you use an open toe type of shoe. When is a foot properly derotated? Unless you can get a coffin bone down to a zero degrees, there is no way of telling, other than the response by the horse. Thruth is that the more lamillar damage occured, the heavier the derotation will weigh.

Long term chronic founder cases that have never been trimmed to lower the palmar angle will end up with a deformed foot that you simply will not be able to take away enough heel off, to significantly lower the PA of P3. Not without weakening the posterior part of the foot so much it will not be able to take any load at all. I did work with counter wedges and banana combos in such cases, but I would not like to have anyone copy this since it's kinda tricky as you can imagine. And it was not so freakin' succesful I feel anyone should copy it either. However it does illustrate the importance of lowering the PA, because only when you have lowered the PA enough you can raise heels.

I have no doubt that all farriers, some of them the very best, that claim raising heels in founder cases is contra indicated have not yet fully understood the importance of lowering the heels before you can attempt to load them. To be able to do that you must derotate/realign, at the very least. Lowering PA beyond zero? You'll not be able to do that, but if you can, do it. But always remember that after derotation/realignment/lowering the PA, you will have increased DDFT pull. It is absolutely vital you counter that, with all you have at your disposal. Dramatic reduction of breakover (way further than an open toe would allow) raising heels, or a tenotomy.

I can not stress the importance of balancing the foot around the COA enough. That goes for all horses, founder cases as well. In a founder case you can even overdo the support length, no problem, but keep protecting the toe as the sole is one of your few allies.
