Pat,

Thanks for posting those photos and commentary.

Why no drain(s)? What, if any, anti-microbal treatment did you use under the foam board?

There was a solar access to this repair. Pouring some MEK (methyl- ethyl ketone) will dissolve the foam under the repair.

As dry as this hoof appears, I would not consider a drain to be absolutely necessary- at least with this type or repair (foam core).

Thanks Pat.

Do you always disolve the foam board after the wall patch has cured? Are you at all concerned about the effect of the MEK on the underlying tissues? In this particular case was the solar access created or the result of a pre-existing abscess track? Did anything further have to be done to keep the access track open during recovery? Was the horse left barefoot? How soon after the surgery was the repair undertaken?

Why was the wall resection done as pictured and how was it determined how much of a window to create?

I apologize for all the questions but I am really interested in the whys and wherefores of this case.

The majority of the time I leave the foam in place. It is an inert material, and just takes up space. My concern for the MEK is offset by the very brief time it is in contact with the hoof, and the small volume required (5cc is more than enough to dissolve a repair this size. I would not use MEK over exposed sensitive tissue, but lightly keratinized tissue like this seems to be OK. The heel was initially floated, but IM was placed under the heel before the end of the first shoeing (no issues, but I like more support if they can tolerate it).

There was no sx, I resected to the borders of the separation. A partial hospital plate was applied after the rebuild, allowing for adjustable amounts and location of frog and sole support.

Pat, in the second picture, what are the holes around the resection for?

Anchor points for the repair, providing more stability.

Pat, that is really inventive for sure. If there is no drainage point, could you fill it up with Keratex hoof putty and copper sulfate, then adhere spectra cloth to seal it?

I haven't had luck packing with with Keratex and then repairing. The copper sulfate and foam seems to work better for me.
