This is a 12 year old quarter horse that I worked on the other day. This is what I saw when I got there. The "other guy" was there 2 days previous.
I could feel heat thru the pad. The horse was non-weight bearing on this foot. I only took pics of his left, but the right looked nearly as bad. She was able to stand on her right, and the sole wasn't so droped.

I felt that I needed to protect the sole, support the back of the foot, and do some major break over adjustment. This is what I came up with. She didn't show much improvement while I was there, but I talked to the owner the next two days, and she said that the next morning the horse was able to walk pretty well, and the next day was even better. I know that it don't look "pretty" but I think that I did what was needed. The breakover point is about an inch back from the toe of the shoe. I used IM in the back 2/3 of the frog. I'll go back in 4 weeks and reset. Hopefully I can make it look better.

Anyone see something that I should have done different?
Thanks.

You really need to get a veterinarian on board that understands hoof anatomy and physiology. The laminae are severely disconnected in this horse resulting in severe flares all the way around the hoof. This is likely a flare up of a chronic laminitis case and can really 'go south' at a moments notice. You need to inform the owner of your concerns and write down everything since the last person who touched the horse may get the blame for all of the problem that has been brewing for months or years.

That is a good temporary stabilization, but this horse will need a lot more in the very near future. Keep him stalled and deeply bedded until the troops arrive.

Dr. Myers,
I told the owner that the horse was in Critial shape, and made no promises. The owner found me thru a vets recommendation, but the vet had not seen the horse in a while. The vet is a good general pratice vet, but doesn't specialize in anything. Dr. Tracy Turner is about 180 miles away, so maybe I'll have to recommend she see him. What might be the next step? What more can be done? You said it was a good temporary stabilization, what would make it more permanent. I know that Dr. Steven O'Grady uses casting matterial after screwing the clogs on. Would that have been better? I think that you are right, it is a reoccuance of founder from one or two years ago, and the shoes that were on the horse were only making the matter worse.
I have had luck with a similar case before. It was a pony, but the first time we set the clogs we had to hold the pony up; 5 months later the owner is trotting there kids up and down the drive way, sound as can be. I know that doesn't mean that they will all be like that.

Thank you for the reply

OMG! That is shocking! Now that I have stated the obvious, can I ask about the clog? Why have you beveled the solar (foot) surface? Is it so you can get IM in there? I have never made one so am just curious. Aslo, how do you keep the IM in the back 2/3s without filling the front with something? What is the process? Nice work if not pretty Mr Upnorth :).

Rachael,
I beveled the foot side because of the dropped sole. I tried to show it in the one pic, it didin't turn out very good, but yo should be able to pick it out now that you are looking for it. Something like a deep seated shoe. When I ran a straight edge across the foot, there was about 3/8 to 1/2 inch to the wall on either side of the sole.
The dental impression material stays in the back of the foot nicely. It forms into the comisures and cleft, and then hardens to about the consistancey of a rubber ball. As long as you have something covering it like a pad, or clog in this case, it shouldn't move around.

Thanks for clarifying. I was somehow thinking the IM was liquid that sets (equipak style) and trying to work out how you would do that with a sealed clog already on said foot - forgot it is more like putty. Sometimes I'm a bit slow! Will you post more pics of next shoeing? Hope it goes well.
