Owch. Something interesting was pointed out to me by one of the vets I used to work woth in California. She *speculates* (is that better, Dr OGrady? ;) )that the bulge in the caudal area of p3 may be bone changes due to stress at the attachment of the DDFT due to long toes or a broken back coffin joint angle.
Heres another one, but obviously dead. And check out the thin thin sole, completely untrimmed! . This horse must hae been in some pain. And I tell you, finding a foot like this in Allie Haye's booth to add to my collection was better than just about anything.

My horse is still alive :p :p :p- "he's not dead yet, but he will be"(Monty Python)
I "speculate" that both these horses had "Navicular Tendancies" and were wedged and barshod for along time without drawing the heels back. Angulation is good but can be detrimental over a long period of time.
Most of these "True Navicular Diseased" feet will show a thin sole and look like someone has taken an ice cream scoop to the bottom of the feet; frog , sole and heels .
This guy had no heel to speak of and was contracted in both front feet in the heels, but the appearance was that of a concaved or scooped out foot. The shoes that came off were GE aluminum wedges and equipak.
I replaced with medium grand champions and Luwex Knobby pads and packed with impression material.

You're right Jaye, often I found that the so called expert reining and pleasure shoers just work towards an overall appearance of feet. They want them to look good. Nicely up, well dressed walls stuff like that.
When you pull those shoes, none of them has a nice base of support. Heels are almost never brought back.
Just try and imagine what happens inside a foot that has heels to much forward, jacked up by a nice egg bar shoe, filled with whatever and the horse is running is heart out. Something has got to give.......... and you know what? It always does, sooner or later.
