I am posting for a freind. She has a 8-9 year old appendix gelding. They have owned him for 4 years. He was purchased with a very good PPE lots of xrays. He passed.

Fast forward to 2 years ago. Pony started going lame in the front. Bilateral seemed like hoof soreness. X rays at that time showed nothing but hoof testers showed and still show caudal food pain. He was taken to UF they found the same thing blocks were done and localized it to the caudal area of his foot. Bone scan showed a tiny uptake in the navicular area but not enough to say conclusive that is navicular. He was put on Isoxsuprine and they were told to shoe him as if he had navicular. Diagnosis waspalmar foot syndrome Was better for a few weeks then became sore in the toe area of the foot. Regular vet decided to inject coffin joints. After coffin joint injections pony was better for a week , Colic surgery forced him to be stall bound one week after injections so the duration of the injections effectiveness was never noted.

The pony is not on any NSAIDS is on and off foot sore one year later. The shoeing that was prescribed by UF has been discontunied per the trainer and farrier since he started getting toe sore. He has not been seen by another specialist in over a year last being at UF. Regular vet and trainer think there is nothing else to be done other than nerve him. . I whole heartedly dissagree.

Please look at his feet and give me your opinion on how he is shod vs could/should be. I think the pony needs to go back to see a lamenss specialist. Also adding he has not had new XRays in over a year and the farrier is not shoeing based on rads I dont think he has ever seen them.

Pony is sore gets worse closer to time to reset he is done every 5 weeks. The flip flop shoes were put on per the farrier...Pony was sound for a few days and then the lammess came back.

I answered it on another thread, you answered your own question; shoe it like a navicular; or if possible if your farrier thinks it is okay for a week; just pull the shoes for now and see what happens, take x-rays again; see if sore still there or not a week later.

TRIM THOSE HEELS!!! .
They are *badly underrun*. No plastic shoe, flip flop, heel bar pad or whatever is going to help if the heels are left overgrown and bent like that.
And he may have a negative palmer angle .(coffin bone sitting lower in the back than the front) so read up on that here, too.

I'm with Patty... The caudal pain is mostly from the heels being underrun and from being so run forward. Those feet need trimmed back for sure. The current package will only exaserbate the situation Because the toe on the shoe will be supported and the heels will sink into the pad. This will make for undesirable ligament functions. JMO

Trim the foot back
Proper breakover setup for the foot
Plenty of support after trimming (and possibly wedging) the heels
You might even need some frog support especially if the zero/neg plane is confirmed.
