does injecting tildren directly above the hoof, like on the pastern, with a tourniquet for like 20 minutes work on a beginning case of pedal bone osteitis?

In a word - no.
Tildren has not been proven to do anything beneficial to the horse. Injecting this product in high concentration into the laminar area could be very damaging.
Pedal bone osteitis is actually the after effect of localized laminitis. The laminae are inflamed resulting in bone resorption in the area. Control and correction of this inflammation is the key to correct treatment. The bone lesion is not due to a metabolic osteoporosis which is the proposed use for tildren.
