Horse loose shoes sometimes with out you being there 24/7 and not seeing how, it is hard to say sometimes.
Overreaching is one of the causes. If some there may be some things your farrier can do to help this. Sometimes there are not depending on the cause.
I would have a talk with your farrier.
Just my thinking.
