Typically the frog points toward the point of breakover ....if that looks off center, so be it....sometimes flares, wry hooves, etc make the foot look like it points in a direction that is incorrect for that particular foot

if the farrier is dealing with a foot like this, the nail holes in a keg shoe will not line up and nail placement might not be what you would expect on a 'normal' foot and make the shoe appear twisted

when I encounter a foot like I think you are describing (with one heel forward of the other), I make sure the heels are of equal length so the landing is even medio-laterally
