Scott,
I would opt out at two things. Either WLD, or an abcess of some kind.
Treatment, if I was sure I was not dealing with an abcess, I would opt for Iodine Patch, like Dr. T. Trunner was using a couple of years ago. If WLD I would leave open and let it grow down. I would shoe with a bar shoe for support and may be some side clips on each side of the hole.
Just my thinking.
